Leicestershire County Council Survey

Leicestershire County Council are seeking people’s views on Community Life Choices services (day services); what is working well and what could be done better. This includes getting the views of people who use the services and their families/carers. The focus is on the commissioning of services rather than the WHM service itself.

The Council have asked us to share the survey with you all- there is one questionnaire for parents/carers and a separate one for workmates.

The survey needs to be completed by Monday 15th January 2024.
